3.8. For each fan, fan coil assembly, fan coil unit, unit heater, unit cooler, air handling unit and
other air moving device listed in 3.6 above, the following shall be included:
a. Identify which items were "on" and which were "secured" during each of the noise
surveys described in 3.1.
b. Identify any items that were required to be operating but were not operational during the
noise survey testing.
c. Identify the HVAC equipment number, compartment location, and system number.
3.9. The noise survey report shall include a list of all non-HVAC equipment that may contribute
to the airborne noise with a given space.
3.10. For each non-HVAC equipment item listed in 3.9 above, the following shall be included:
a. Identify which items were "on" and which were "secured" during each of the noise
surveys described in 3.1.
b. Identify any items that were required to be operating but were not operational during the
noise survey testing.
c. Identify the equipment number, compartment location, and system number.
3.11. The noise survey report shall include the following:
a. For each compartment and on-deck work station, the maximum measured noise level,
assigned noise limit, and noise excess;
b. Compartments and on-deck stations experiencing hazardous noise conditions;
c. Compartments and on-deck stations experiencing noise levels in excess of 140 dBP; and
d. The specific machinery and equipment items, HVAC systems, and high noise level
compartments and areas responsible for the detected noise excesses, and the transmission
and radiation paths controlling airborne or structureborne noise.
3.12. If measured noise excesses exist, the noise survey report shall include a set of noise
control options that could be implemented to eliminate the noise excesses detected.
